Primary Partners Accountable Care Organizations (ACO), participated in the CMS Medicare Shared Savings Program from 2012-2021. Our Medicare ACO received the highest quality score in Florida in 2014, and 4th in the nation. In 2015, our quality score remained #1 in Florida, scoring 99.2%.
From 2022 to end of 2023, Primary Partners transitioned into a Direct Contracting Entity (DCE) model. In this model, participating physicians are rewarded for their efforts and dedication to providing exceptional quality care, while also controlling rising healthcare costs. Our providers’ fee-for-service Medicare patients benefited from receiving robust, patient-focused, well-coordinated healthcare. Whether you participate as a physician or patient, you’ll benefit from our programs.
Primary Partners connects our providers with major commercial health insurance companies by offering “value contracts.” Through these programs, patients have access to our network of highly responsive doctors as well as unique programs, like chronic care management.
Similar to our ACO, physicians are rewarded for their efforts and dedication to providing exceptional quality care while also controlling rising healthcare costs.
Primary Partners has extended its population health services to Medicare patients through risk-based agreements with select Medicare Advantage plans. Primary Partners will be responsible for the complete continuum of healthcare for certain patients who select a primary care physician affiliated with the group. This model helps prevent costly health episodes that result in hospitalization – and it extends Primary Partners’ reach further.
Primary Partners also offers Direct Primary Care. In this model, patients pay a monthly fee to access care directly from their primary care physicians, which includes check-ups, office visits, 24 hour access to care, and some basic lab services. Other services such as lab tests, and diagnostic services are available through outside providers at discounted prices. Patients can be assured of and enjoy a strong physician-patient relationship. This model empowers our patients to achieve better health, and it represents Primary Partners’ traditional values in patient care.
Primary Partners’ physicians also make up the PCP & Specialist network for regional, self-insured employer groups. With these arrangements, our doctors provide the highest quality, well-coordinated, cost-effective care funded through insurance premiums paid by self-insured groups.
